This study is part of the study funded by the Fundamental Research Grant Scheme (FRGS) entitled "Elucidating The Prevalence, Determinants And Novel Mechanism For COVID-19 Vaccination Hesitancy Among Healthcare Workers in Malaysia" from the Ministry of Higher Education (203.PPSP.6171313). This data consists of two sets of data which are i) Exploratory Factor Analysis (EFA) and ii) Confirmatory Factor Analysis (CFA). Methods The questionnaire was developed based on the Health Belief Model, literature reviews, and discussions with research team members. A step-by-step validation procedure involving content, face, and construct validity was applied. The construct validation was conducted via two cross-sectional studies. Initially, the validation was performed among 125 HCWs using EFA, and then the validation was conducted among 300 HCWs using CFA. The study was conducted via both face-to-face and online platforms. Results A high item Content and Face Validity Index are displayed by the preliminary ACBD, which has 22 items. The EFA analysis demonstrated three domains with 20 items. The final questionnaire after CFA analysis consists of three domains with 16 items. The level of goodness of fit (comparative fit index = 0.956, Tucker-Lewis index = 0.946, incremental fit index = 0.931, chi-squared/degree of freedom = 2.669, and root mean square error of approximation = 0.075), signifying a fit model. The Cronbach’s alpha of this questionnaire is 0.91.
